2 In Chile the survey was conducted in Antofagasta, Antonio, Arica, Calama, Chiguayante, Chillán, Concepción, Copiapó, Coquimbo, Coronel, Iquique, La Serena, Linares, Los Angeles, Lota, Osorno, Ovalle, Puerto Montt, Quillota, Quilpue, Rancagua, San Curicó, Santiago, Talca, Talcahuano, Temuco, Valdivia, Valparaíso,Villa Alemana, and Viña, representing 70% of the adult population.
3 In China the survey was conducted in Beijing, Chengdu, Guangzhou, Hangzhou, Shanghai, Shenyang, Wuhan, Xi’an, and Zhengzhou, representing 36% of the total urban population.
4 In Costa Rica the survey was conducted in Alajuela, Cartago, Heredia, Liberia, Limón, Nicoya y Cañas, Puntarenas, Resto del AMSJ, San Isidro de Peréz Zeledón, and San José Central, representing 100% of the total urban population.
5 In El Salvador the survey was conducted in Apopa, Delgado, Ilopango, Mejicanos, Nueva San Salvador, Santa Ana, San Martin, San Miguel, San Salvador, and Soyapango, representing 100% of the total urban population.
6 In Guatemala the survey was conducted in Chimaltenango, Cobán, Escuintla, Guatemala, Huehuetenango, Mazatenango, Mixto, San Marcos, Villa Nueva, and Xela, representing 71.5% of the total urban population.
7 In Honduras the survey was conducted in Choloma, Comayagua, Distrito Central, El Progreso, La Ceiba, La Lima, Puerto Cortes, San Pedro Sula, Tela, and Villanueva, representing 100% of the total urban population.
8 In India the survey was conducted in Chennai, Delhi, Kolkata, and Mumbai, representing 5% of the total population.
9 In Indonesia the survey was conducted in Jakarta and Surabaya, representing 13% of the total urban population.
10 In Nicaragua the survey was conducted in the metropolitan area of Managua and the Capital departments of Chinandega, Granada, León, and Matagalpa, representing 100% of the total urban population.
11 In Panama the survey was conducted in Chiriqui, Cocle, Colon, Herrera, Los Santos, Panama, and Veraguas, representing 100% of the total urban population.
12 In the Philippines the survey was conducted in the National Capital Region, representing 27% of the total urban population.
